We respect your privacy and want to ensure the personal information you have provided to us remains accurate, confidential and secure. We provide your personal information only to those within Sexton Group Ltd and Cascadia Motivation who need it for the purposes associated with the Program.

You will need to have the following information with you for all passengers travelling to complete the registration:

• Full legal names (as it appears on your valid passport).
• Birthdates.
• Passport numbers and expiry dates (if you are in the process of applying for a new passport, please put “pending” in this box on the registration).
• A passport valid for 6 months after your return is required for entry to Costa Rica. It is your personal responsibility to ensure you have the proper documentation.
• If you are not a Canadian citizen: Permanent residents of Canada will require a permanent resident card to gain entry back into Canada.
• International Customs & Immigration have become very rigid in their entrance requirements. Anyone having any major or minor criminal offence should check with Immigration, as you may be denied entry upon check-in at the airport.

We request a copy of your passport because several times every year participants lose or misplace their passports or provide incorrect details that could inhibit their travel.
